I hereby give the below named registrant/participant permission to attend the "Soccer Dynamics Summer Programs" coordinated by Soccer Dynamics. I verify to best of my knowledge that the below named registrant /participant is both physically and mentally able to participate in the activities of a soccer program and has been examined and given the approval by a qualified physician to do so. Recognizing the possibility of physical injury associated with soccer and in consideration for Soccer Dynamics accepting this registrant/participant for its soccer programs and activities, I hereby release, discharge and/or otherwise indemnify Soccer Dynamics, its employess and personnel, including the owners of the fields utilized for the programs, against any claim or damages to person, property or, activities by or on behalf of the registrant's/participant's participation in the program and agree to assume all financial responsibility including all medical/dental treatment including possible insurance deductibles and/or reasonable attorney's fees, made by any third party due to or arising out of your participation in the programs or by violation of any term of this Liability Waiver. I Agree to allow registrant/participant to be treated, if necessary, by a physician/dentist and/or physical trainer while attending the program. I give permission to Soccer Dynamics to use any/all program training photos, videos and/or comments in its publications, brochures and/or websites, printed or electronic.
